Please note: THESE ARE NOT REGISTRATION FORMS.

You must use Morehead's online registration system to register your child for camp. Do not complete and submit these forms until AFTER you have registered your child through the online registration system.

To confirm your child's registration for Morehead's Summer Science Camps, you must complete and submit several forms within two weeks of your online registration. Your child cannot attend camp unless these forms are completed, signed and returned.

When you receive your registration confirmation by email, you will also receive these forms as attachments to the email. If you have completed online registration but have not received your registration confirmation, please check your email spam filter to be sure that your registration confirmation wasn't blocked by the spam filter.

Print, complete and sign all of the forms and return them to Morehead at this address:

You may also fax the forms to Morehead at 919-962-1238; scan the forms and email them to mpsc_camps@unc.edu; or hand-deliver the forms during regular business hours.

Morehead reserves the right to cancel a registration if these forms are not submitted within two weeks of that registration.
